Planned Absence Request Form

This form should be used for absences of 3 days or more. By completing this form prior to planned absence, dates will be marked as excused.

There may be circumstances when events take place during the school year causing a student to miss school. On these occasions, it is the responsibility of the parent and student to complete a “Planned Absence Request” form. This should be completed at least 2-3 days prior to the absence.
